Interface designer / usability specialist job applicant's test task

Our company specialises in developing web-based applications. Those applications consist of html or flash forms used to enter data, and reports to display the data.

The work of the interface designer will involve designing web application interfaces based on given technical specifications. The forms should be usable and neat.

This task is for those applying for a web interface designer / usability specialist job:

Take the form featured below and suggest the ways to improve it.

Deliverables:

 1. An improved form in html, flash or graphic format.
 2. A document with the following structure:
    - Assumptions: explain the assumptions you make about web site users.
    - Problems: Describe the problems with the current interface.
    - Solution: What is the rationale behind your solution?
    - Ideas: Other ideas, suggestions and comments.

Both visual and the document with explanations are of equal importance. We need a designer who can explain what s/he has done and why, in good written English.

Do not think that this is a simple task – there is a lot that can be done with this form. If you cannot see any ways to improve it - this job might be not for you.

There will be a lot of applicants as we have advertised in several places, so we cannot answer all applications individually. But do not be discouraged by that! From our experience, 90% of candidates simply do not answer the brief, even if it is a short one like this. If you are serious, invest a little of your time and you will increase the odds in your favour. However, do not spend more then 2 hours on this task as we are looking for efficiency as well as talent.
